The Oklahoma Compassionate Care Campaign Endorsement Form

We are asking patients, doctors and/or organizations to endorse this simple statement:

* “My organization or I endorse the efforts of the Drug Policy Forum of Oklahoma
to pass medical marijuana legislation in Oklahoma.”

* We have received many anonymous endorsements (and they do help), but a personalized
endorsement is a much more powerful contribution to this campaign.
